Among people in prisons and people who inject drugs, two groups that have historically experienced significantly higher HIV burdens than the general population, prevalence rates continue to decline. Between 2010 and the most recent reporting period, prevalence among people who inject drugs dropped from 15% to approximately 1.2%. 

Among people in prisons, prevalence fell from 2.1% in 2009 to about 0.8% in recent years. These gains are largely attributed to Iran’s robust harm reduction programs, including widespread access to needle-syringe exchange services and opioid substitution therapy.

The Global Fund is the largest multilateral donor supporting national health system strengthening in low- and middle-income countries, investing over USD 5 billion annually to combat HIV, TB, and malaria while strengthening health systems and pandemic preparedness. Since 2002, it has disbursed over USD 65 billion across more than 100 countries, aiming to defeat infectious diseases and promote health equity.
